Leadership Institute Application

To Apply:
Enrollment is limited to a maximum of 50 people and is by competitive application only.

Step 1: Complete the application online here. CSDC suggests you allow yourself 30-45 minutes to complete the application.

Step 2: Download the commitment statement here. Sign and return with Institute fee.
(Checks will not be deposited until acceptance and attendance are confirmed by CSDC and applicant. However, applications will not be considered without accompanying payment.)

Step 3: Mail completed commitment statement and $650 Institute fee to:

Only complete applications (both parts completed and submitted),
with full payment, will be considered.

Application deadlines:

Early application deadline: March 28
Regular application deadline: April 25

Acceptance Notification:
Confirmed attendees will be notified and sent additional information after they are accepted. We strongly encourage applicants with culturally diverse backgrounds and/or those who will be leading schools serving less advantaged students. If you have any questions about the application, please do not hesitate to contact us.

If necessary, enrollment may also be limited to one or two representatives per school. If we are unable to accommodate your registration, your check will be returned promptly.

Cancellation Policy:
The cancellation policy applies to those individuals who cancel after they have already been accepted and have committed to the institute. Those who cancel before June 6, 2008 will be refunded the total minus a $100 cancellation fee; however, those who cancel on or after June 6, 2008 will not be reimbursed. Those who do not attend the Institute will not be eligible to receive Institute materials. If you have any questions about this policy, please contact CSDC.

Withdrawal Policy:
The withdrawal policy applies to those individuals who cancel before they have been accepted or while they are on a waiting list. Those who cancel before the regular application deadline of April 25, 2008 will be returned the original check they submitted. Those who cancel while on a waiting list will be returned the original check they submitted.
